Youth sports specialization is on the rise. As a result, the prevalence of pediatric ACL injuries is also increasing. Join Columbia University Sports Medicine on November 13th for Pediatric Sports Injuries: Treating our youth from head to toe to learn about the latest techniques for prevention and treatment of these injuries in our young athletes.

Children are playing sports in record numbers across the United States, and with this increase in participation there has been an alarming rise in the number of sports injuries. Taking optimal care of these injured kids requires extensive knowledge and comfort in techniques that have previously been thought of as only pediatric, or only adult. This webinar reviews and discusses treatment strategies to maximize outcomes and get these developing athletes off the sidelines and back onto the field. Understanding the etiology, diagnosis, surgical, and non-surgical modalities are the key elements of a successful treatment plan. This course will be of interest to Team Physicians, Certified Athletic Trainers, Physical Therapists, Primary Care Sports Medicine Physicians and coaches of all youth levels.

At the conclusion of this activity, participants will be better able to:

  • Interpret and differentiate the physiology of pediatric sports injuries to formulate a treatment plan
  • Classify and identify different graft options to reconstruct a pediatric ACL injury in order to generate an appropriate recovery plan

Understanding Tommy John Surgery – And How To Avoid It

Dr. Christopher AhmadIn baseball, we have reached epidemic levels of players needing Tommy John Surgery. At the Major League level, one in four pitchers has had Tommy John as a result of tearing their ulnar collateral ligament, or UCL. Unfortunately, these numbers translate to many younger players needing the surgery as well. To help combat the issues facing America’s pastime, I, along with Frank Alexander and Charlie Ahmad put our heads together and wrote Understanding Tommy John Surgery and How To Avoid It: A Guide for Young Baseball Players.

Understanding Tommy John SurgeryTo reverse these epidemic trends our goal is to start changing the culture at the youth levels before bad habits develop, similar to how we educate children on the dangers of smoking. In our office, we’re seeing younger and younger players walk through the door at an alarming rate. We hope that by educating younger athletes, they won’t wind up in our office when they’re older.

They viewed it as a rite of passage. No player should ever want to have surgery and it’s time that we change that line of thinking.”

How To Help Prevent Overuse Injuries

Frank AlexanderIf you play competitive sports, injuries are always a possibility. Nobody knows exactly when or how they will get hurt. Unfortunately, there is no way to predict injuries. Even the most elite athletes fall victim to injuries and spend time on the disabled list or have to sit a few matches out.

While we cannot predict much about injuries, we can avoid many of them. All athletes can prevent a group of injuries called “overuse” injuries. Overuse injuries are just that – when you over use a body part such as an elbow, shoulder, or knee without adequate rest.

golf swingMany overuse injuries happen in adolescent athletes that are still growing. These injuries are fairly common and you may have even heard of a few such as: Little Leaguer’s Shoulder, Little Leaguer’s Elbow, or Jumper’s Knee. Adolescent athletes are not the only population that succumb to overuse injuries. Weekend Warriors, runners, frequent golfers and tennis players also develop overuse injuries. Common overuse injuries in the skeletally mature population include: Tennis Elbow, Golfer’s Elbow, and IT Band Friction Syndrome.

Nobody wants to hear they are injured but the good news is you can recover from them. You have an excellent chance of making a full recovery if you let your body rest and correct any asymmetries and muscle imbalances. Many athletes will not want to “rest” but rest does not mean you cannot exercise or play sports. It just means you have to adjust your routine.

tennis elbowAthletes that have overuse injuries can still participate in sports but may have to stop the sport they got injured playing. If you have Little Leaguer’s Shoulder, it doesn’t mean you have to stop everything for the whole spring or summer, it just means you should take a break from throwing a baseball. Depending on the severity of the condition, some athletes may be allowed to play basketball, soccer, or even swim in a pool. For patients suffering from Golfer’s or Tennis Elbow, a simple adjustment of how you hold a club or a racket may be all it takes to alleviate your symptoms. Any runner with ITB Friction Syndrome may have to take a break from running, but they can still get in the pool or ride a bike. There are plenty of ways to make minor adjustments to exercise routines and still be active while injured!

The best way to prevent these injuries is to make sure you give your body a break every now and then from the demands you place on it. This is especially important for our young and growing athletes. Baseball and tennis players should give themselves a break from throwing or competing after their season ends. Runners can hop on a bike or elliptical every once in a while to avoid injury as well. There are many ways to avoid overuse injuries so if you think you may be prone to them, feel free to ask any expert on our Team and we’ll be happy to help you stay on the court, field, or track!

Orthopedics Today Round Table Discussion On Tommy John Surgery

Dr. Christopher AhmadSerious arm injuries are happening to baseball players at the youth, high school, college and professional levels at an alarming rate. Just this week, the eyes of the professional baseball world turned toward the St. Louis Cardinals #1 pitching prospect Alex Reyes underwent season-ending Tommy John surgery.

I was privileged to serve as the moderator on the Orthopedics Today Round Table with a panel that included Dr. James Andrews, Dr. Michael Ciccotti, Steve Donohue (trainer, New York Yankees), and others.

Steve Hayward On “The Sports Doctor” Radio Show

Baseball Health Network founder Steve Hayward brought the BHN story to a national audience Steve Hayward - Baseball Health Networkwith his January 4, 2017 appearance on “The Sports Doctor” Internet radio show hosted by Dr. Robert Weil.

Steve and Dr. Weil talked about the history of the BHN, the epidemic of arm issues in baseball, especially at the youth level, the bad rap curve balls are getting, and how both parents and players need to be pro-active with regard to the physical health of athletes.

Baseball Health Network Seminar May 9, 2016

Baseball Health NetworkThe staff of the Baseball Health Network was invited by the Englewood Cliffs (NJ) Little League to speak in front of coaches, parents and players at the Englewood Cliffs Upperschool on Monday, May 9.

The evening included presentations by BHN founder Steve Hayward, Performance Director Eric D’Agati, and BHN founding partner Doug Cinnella.

Eric‘s demonstration on Trunk Rotation and Shoulder Mobility, using a local high coach as his test subject is always enlightening. The amount of improvement in mobility in just a few minutes is certainly eye-opening.

Steve and Doug concentrated on simplifying the pitching process. All too often young pitchers can get completely overwhelmed by the task at hand – throwing strikes.

Doug Cinnella Baseball Health NetworkSteve detailed a couple of drills designed to get pitchers lined up straight to their target and Doug‘s “step-step-throw” demonstration showed how pitchers can work not only on getting their bodies going straight, but also finding their natural arm slot.

Why Are Kids Leaving Baseball?

youth baseballIf you’re involved with youth baseball at any level, odds are you saw a post recently on social media by Darren Gurney. A long-time high school coach in Westchester County, NY, Darren went into a number of reasons why he believes that baseball is losing younger players and fans.

As a coach on the front lines of youth baseball, this article certainly struck a chord with me, as I have seen not only the participation numbers, but also the quality of play, in youth baseball rapidly decline over the past 10 years of coaching and seeing my two sons play. The prime of youth baseball is between the ages of 10-12 years old, when players participate is what is usually called the “Majors” level in Little League and Cal Ripken leagues. This is the age bracket that millions tune in to watch each summer compete in the Little League World Series. Although ratings for this event have risen to epic levels, what I have seen first hand at the local level is sad to see as a lifetime baseball enthusiast.

My local town now has exactly half of the number of teams in the Majors that it had just 5 years ago when my older son started playing at that level. The only reason that they even have that number of players is that most town/recreation organizations also have affiliated town travel teams and they require the travel players to also play “Rec” baseball in order to play travel.

Although the above-mentioned article touches on many of the reasons that this drop off has occurred, let’s investigate the issue further. For the purposes of this discussion, we will focus on “Rec” or “Town Team” baseball and leave the world of travel, club team and showcase baseball for another time.

“Two-Class Society”
On any recreational team, you will have a clear distinction between two types of players, ones that play on travel teams and ones that don’t. The ones that also play travel more than likely started doing baseball-specific training back in January at an indoor facility with their respective team and/or getting individual training in pitching, hitting and/or fielding. The non-travel players most likely won’t take their glove and bat (if they even own one) out of their bag until the first day of practice in April.

14U pitcherBecause of the superior skill set and training background that the travel players have, it leads to an interesting dilemma when it comes time for the Rec team coach to make the line up and determine where each player will play. As much as we would like to be idealistic and let kids try to play the “premier” positions like pitcher or infielder, it now becomes a safety issue as the less skilled non-travel players may not be equipped to defend themselves when a talented and powerful travel player uses one of their high-powered (and high cost) composite bats to launch a screaming line drive back towards them. In the name of self-preservation, as coaches we will tend to put these players in the outfield where it is safer.

 

“Games, Games and More Games… and maybe a practice”
Most recreational/town leagues will have a 14-18 game schedule in the Spring season. Because they are waiting to maximize registration, they hold off on forming teams until all of the potential players have finished their winter sport season, which usually is around the first week of April. The season is usually scheduled to start two weeks later. With school Spring breaks, rainy April days and limited town field availability, there are usually limited opportunities to allow each team to practice prior to getting started. Once the season starts, there are two games per week, and with the travel players being involved in their other team’s practices and games at least 2-3 days per week, as well as field time being at a premium, it is nearly impossible to hold practices during the season. And as the other article mentioned, very rarely do you drive past field and see kids just having a pick-up game with their buddies, so the only time they play baseball is when it is organized and scheduled.

youth practiceIt finally hit me when I was coaching my Rec team one year and I realized, “I really can’t make these kids better even if I wanted to.” Without practice time, I get maybe 30 minutes prior to a game that I can get in a batting cage, where each player gets maybe 12-15 swings each. If we are lucky before our game starts, we may get 5 minutes to do a quick round infield/outfield practice. Other than that, it’s real tough to teach the intricacies of this great sport between pitches and innings during the game. And remember, the non-travel players generally don’t touch their glove or bat until they show up for the game.

So, now let’s summarize. If I’m a non-travel player, I get stuck in the outfield, which is boring, and usually have to sit out a few innings. If I do get one ball hit to me, I have little chance of making a play since the only balls hit to me I’ve seen all week were the two the coach hit in warm-ups. Since the only batting practice I get is the dozen swings before the game, I also have little chance of getting a hit, especially if one of the travel kids is pitching. So at the end of the day, I sat 3 innings, got stuck in right field and made 2 errors and struck out 3 times. Where do I sign up for lacrosse or soccer???

youth practiceIf we are going to have any chance of saving this game we need to cut back on the games and get more practice time. In a practice, these players can now get 50 swings and catch 50 ground balls and fly balls. As they gain skill, not only can they safely be used in better positions, their likelihood for success dramatically increases, as does their love for the game. If you had one practice per week like this, you could still have one game per week that would not only have more meaning, but they would be more competitive and have a much more engaged group of players that will tell their friends how great baseball is.

I remember having this argument with a league official as he was telling me how one level was set up to be “instructional” and another was “competitive”. My response was if you want something “instructional”, that is called practice. If you want something called “competitive”, it’s called a game. Either way, whatever we are doing now is not working and hopefully we can change it before the sport goes away completely.
